How to obtain a WWW account

You can either download, print and follow the instructions in the Postscript version, the ASCII version, or use the following form.

We need a user name for the account. Your documents will show up starting at the url http://www.phone.net/home/user. That is also the name for your ftp account, and your mail drop as well.

We need an electronic mail address. Electronic statements will be sent to that address, as well as all mail to the user name that arrive at phone.net.

We need to know a real name for the account. This will also be the name that shows up on our customer list.

We need an address for surface mail, which we hope to use only rarely. Only the information entered in this input form will be used. If you need the real name entered above to appear in the mail, you must reenter it here.

Finally, we need to know that you've downloaded and read the license, and a agree to it's terms.
